Introduction: Supernumerary teeth are defined as a developmental disorder characterized by the presence of one or more dental elements outside the number considered normal in an arch. Its presence can cause a series of consequences, including the formation of cysts and root resorption, diastemas, crowding, delayed eruption and impaction of adjacent teeth. Diagnosis of this element involves routine clinical and radiographic examination. Objective: To report a clinical case of prolonged retention of a deciduous tooth and impaction of a permanent tooth due to the presence of an impacted supernumerary element. Case Report: After identifying the mesiodens through analysis of radiographic exams and computed tomography, surgery was performed to remove the deciduous and supernumerary elements, and the bonding of an orthodontic device to pull the permanent element. Results: The surgical intervention was performed in conjunction with button bonding for orthodontic traction, which provides greater chances of successful eruption of the permanent tooth. There were no postoperative complications and after a week the patient returned to have the suture removed and was referred to the Orthodontics Specialization Center of the Faculty of Dentistry of the University of Itaúna for clinical follow-up and to continue the treatment. Conclusion: The timely detection and removal of supernumerary elements are important to allow aesthetic and functional harmony and avoid future damage to patients' occlusion.
